- as the data subject, I have the right to access and rectify my data,

I have been informed about my rights arising from Section 32(1) of the above-mentioned Act.

I give my consent to receiving commercial information relating to products and services of Marmed Health Care Sp. z o.o. and its collaborating entities, by means of electronic and postal communication within the meaning of the Act of 18 July 2002 on Provision of Services by Electronic Means (i.e. “Dziennik Ustaw” [Journal of Laws] of 2013, item 1422, as amended). Data provision is voluntary. I have the right to access and rectify my data.

I give my consent to receiving voice messages constituting/containing commercial information relating to products and services of Marmed Health Care Sp. z o.o., generated by automated calling systems (so-called voice sms) within the meaning of Section 172(1) of the Telecommunications Law Act of 16 July 2004 (i.e. “Dziennik Ustaw” [Journal of Laws] of 2014, item 243, as amended).
